• Gathers information for intake assessment and housing placement. Conducts intake interviews and completes individual assessments of client’s status and level of required comprehensive services.
• Prepares and maintains assessments, comprehensive service plans, progress notes and other records and documentation.
• Promotes Housing First ideologies, acts as a liaison between landlord and tenant.
• Makes appropriate referrals to community providers coordinating and arranging access to care with ancillary services.
• Facilitates placement including arranging pre-placement visits and transportation, or provides transportation as needed.
• Conducts chart and treatment plan reviews to ensure compliance with agency, federal, state and local government standards.
• Monitors and evaluates client progress and changing needs throughout treatment.
• Develops and maintains a caseload as assigned by the Program Manager, and maintains a daily log of client contacts.
• Provides community outreach and early intervention activities by supplying information about the program to community agencies connected to the SUD services system in order to educate them about program availability.
• Provides support to clients and families and encourages family members and client significant others to participate in client stabilization and counseling services refers those needing specific services and support.
• Maintains contact with assigned treatment providers to continuously appraise client progress, and provides crisis intervention.
• Promotes effective relationships with staff by demonstrating a coordinated team approach to treatment, and prepares critical care issues for discussion during bi-weekly supervision.
• Establishes weekly case presentations for admission review, including all admissions in the caseload.
• Intervenes for non-open cases in a crisis situation by referring to mental health services.
• Informs at-risk populations about the need for and availability of Medi-Cal and non-Medi-Cal mental health programs.
• Provides telephone, walk-in, or drop-in services for referring persons to Medi-Cal and non-Medi-Cal mental health programs.

• Possess or be in the process of obtaining certification for substance abuse treatment as CAADAC, CASS or similar professional.
• Two years of providing substance abuse treatment and care to adults.
• In-depth knowledge of substance abuse treatment methodologies and diagnostic techniques. Familiarity with San Diego substance abuse services system.
