    Position Description

    • Completes all required forms. Records and reports any significant behavior from donor during collection process. Ensures restroom and work area is clean and sanitary at all times.
    • Maintains confidentiality at all times.
    • Identifies and corrects chain of custody documents, corrects and resolves discrepancies, verifies accuracy and completeness of drug testing and control forms.
    • Verifies specimen bottles are properly sealed and shipped to laboratory per regulations. Explains collection process to donors. Displays regular and reliable attendance.
    • Reports to and departs from work on time, as scheduled, and accurately reflects all time on time card.

    Education/Experience Requirements

    • High School diploma or GED.
    • Previous experience with urine collection.
