FAMILIES FORWARD PROGRAM: Families Forward Wraparound provides mental health and supportive services with a team of caring professionals. Our program helps children and youth remain in the home with their families, and assists them in becoming more successful in other key areas of their lives. Our wraparound process honors the family’s voices and brings people together from different parts of the family’s life to provide a network of support. Families Forward Wraparound services are creative, culturally inclusive, flexible and action-oriented.
REPORTING RELATIONSHIP: This position reports to the Regional Supervisor.
POSITION PURPOSE: The Wrap Facilitator will develop a supportive and helping relationship with assigned high-risk youth and their families that follows the Wraparound philosophy and approach. The Facilitator will ensure that clients receive the full spectrum of services in a seamless manner and that they receive the optimal level of care needed to maximize functioning and minimize the need for higher levels of care. The Facilitator is responsible for overseeing the clinical services provided and is responsible for the client’s medical record. They serve as the hub of the team and complete intake, behavioral health and discharge assessments.
