  • Program/Department Description

    North Coastal Mental Health Center serves young adults, adults and older adults with severe and persistent mental illness who are uninsured, indigent or have MediCal and/or Medicare insurance. We partner with our clients in their recovery to assist them in achieving their highest possible level of functioning with the goal of transitioning from our clinic to primary care providers.  We strive to offer the highest quality services in a manner that is gender responsive and culturally sensitive.

    Position Description

    The Clinical Case Manager coordinates care for clients and assists them in accessing services and resources that will help maintain them in the community in the least restrictive environment. In addition, the Clinical Case Manager acts as a liaison between treatment providers and the client and assists clients with the development of social and community support systems that promote stability and enable clients to function at the highest level possible. The Clinical Case Manager offers extensive knowledge of community resources and support as a primary means of assisting each client to reach their rehabilitation and recovery goals.



    • Coordinates the full range of client’s needs. Provides brokerage, linkage and advocacy to ensure resources and services are accessed in a timely manner.
    • Conducts individual counseling including crisis intervention, and oversees psychoeducational groups.
    • Complies with all charting and documentation requirements per contract, funding agency, and MHS policy
    • Develops and maintains an appropriate caseload.
    • Collaborates with ancillary service providers in the fields of education, employment, housing and other social services.
    • Serve in family advocacy role to assist family members with client’s rehabilitation.
    • Completes all charting and documentation as contractually required and in timely manner as dictated by program requirements.
    • Maintains contact with assigned treatment providers to continuously appraise client progress.
    • Prepares critical care issues for discussion during supervision and treatment team meetings.
    • Displays regular and reliable attendance. Reports to and departs from work on time, as scheduled.
    • Successfully functions as a team member.
    • Complies with all MHS and program rules, policies and procedures including HIPAA standards.


    Education/Experience Requirements

    • Bachelor’s degree in a behavioral science and at least two years experience providing services to individuals with mental illness.
    • Spanish speaking preferred.
    • Experience providing care to acutely mentally ill adults preferred.
