  • Program/Department Description

    The Ontario One-Stop Transitional Age Youth (TAY) Center offers young adults between the ages of 16 to 25 a safe, accepting and comfortable place. Young adults come to identify the TAY Center as their own safe haven, wher they can define and succeed in accomplishing their life goals.  The young adult participants often have mental, emotional and/or substance abuse concerns.  They come from foster care, group homes, the juvenile justice system, living on the streets, or simply from the community at large.  They come to fulfill basic life needs such as a warm meal, a shower or a place to do laundry.  After they drop in, they find additional supportive services, such as counseling, job training, housing referrals, GED study classes and other enrichment courses. 

    Position Description

    Case Managers act as liaison between treatment providers and the client. Case Managers are responsible for intake and client need assessment, arrangement and coordination of comprehensive services, referral to AOD treatment and recovery services, monitoring and evaluation of client progress and changing client needs and client advocacy. The primary task of the Case Manager is assisting clients with the development of social and community support systems that provide treatment and recovery opportunities promote stability and enable the client the opportunity to find and continue employment. Case Managers offer their community outreach abilities and extensive knowledge of community resources and support as a primary means of assisting each client with achieving long-term recovery and employment. This position will also serve as a backup Psychiatric Technician, as needed, providing care to mentally ill, emotionally disturbed, or mentally retarded patients participating in relevant treatment activities / programs. Will assist patients with their personal health regimes, educate and encourage the correct use of oral medications and hypodermic injections, following physician's prescriptions. Will take and record measures of patient's general physical condition, such as pulse, temperature, and respiration, to provide baseline or daily information.

     

    ESSENTIAL DUTIES AND RESPONSIBILITIES:

    • Gathers information for intake assessment. Conducts intake interviews and completes individual assessments of client’s status and level of required comprehensive services.
    • Prepares and maintains assessments, comprehensive service plans, progress notes and other records and documentation.
    • Makes appropriate referrals to community providers coordinating and arranging proposed treatment placements with providers. Facilitates placement including arranging pre-placement visits and transportation.
    • Conducts chart and treatment plan reviews to ensure compliance with agency, federal, state and local government standards. Monitors and evaluates client progress and changing needs throughout treatment.
    • Develops and maintains a caseload as assigned by the Program Manager.
    • Provides community outreach and early intervention activities by supplying information about the program to community agencies connected to the AOD services system in order to educate them about program availability.
    • Provides support to clients and families and encourages family members and significant others to participate in client stabilization and counseling services and refer those needing specific services and support.
    • Provides crisis intervention.
    • Transports clients when necessary.
    • Maintains appropriate documentation on all clients as required by contract and as outlined in the program procedures manual.
    • Maintains contact with assigned treatment providers to continuously appraise client progress.
    • Promotes effective relationships with staff by demonstrating a coordinated team approach to treatment.
    • Prepares critical care issues for discussion during bi-weekly supervision.
    • Establishes weekly case presentations for admission review. This process includes all admissions in the caseload.
    • Provides input on an ongoing basis to develop and improve program servicing and delivery.
    • Displays regular and reliable attendance. Reports to and departs from work on time, as scheduled.
    • Successfully functions as a team member.May participate in restraining or subduing agitated patients.
    • Serve as backup Psychiatric Technician, as needed, providing professional and personal care for emotionally disturbed consumers, and aids in the effective and proper use of previously prescribed medications, observing and evaluating patients' behavior, recognizing symptoms and potential violence of patients, assisting patients in meeting hygiene and nutritional needs.
    • May participate in restraining or subduing agitated patients.

    Education/Experience Requirements

    EDUCATION and WORK EXPERIENCE:

    • High School Diploma, GED equivalent.
    • Must possess and maintain a current Psychiatric Technician License issued by the California Board of Vocational Nurse and Psychiatric Technician Examiners.
    • Two years of providing substance abuse treatment and care to adults.
    • In-depth knowledge of substance abuse treatment methodologies and diagnostic techniques.
    • Familiarity with San Diego substance abuse services system.
    • Must be bilingual, English and Spanish speaking.
