    FAMILIES FORWARD PROGRAM: Families Forward Wraparound provides mental health and supportive services with a team of caring professionals. Our program helps children and youth remain in the home with their families, and assists them in becoming more successful in other key areas of their lives. Our wraparound process honors the family’s voices and brings people together from different parts of the family’s life to provide a network of support. Families Forward Wraparound services are creative, culturally inclusive, flexible and action-oriented.

    Position Description

    The Wrap Facilitator will develop a supportive and helping relationship with assigned high-risk youth and their families that follows the Wraparound philosophy and approach. The Facilitator will ensure that clients receive the full spectrum of services in a seamless manner and that they receive the optimal level of care needed to maximize functioning and minimize the need for higher levels of care. The Facilitator is responsible for overseeing the clinical services provided and is responsible for the client’s medical record. They serve as the hub of the team and complete intake, behavioral health and discharge assessments.



    • In collaboration with the client’s natural team members and family, serves as the main contact of the Wraparound process.
    • Facilitates Wraparound meetings with other program staff, clients, families, informal supports, and formal supports to ensure fidelity to Wraparound planning process.
    • Provides strength-based, individual and family or group mental health services, in the home, community, or on site.
    • Provides clinical assessment of youth, completes all County and program required assessment documentation in the Electronic Health Record.
    • Collaboratively ensures the development of the Individualized Child and Family Team Plans.
    • Conducts face to face meetings and makes initial contact with clients.
    • Designs, plans, organizes and supervises clinical procedures with respect to casework.
    • Provides case management services for the client and family in the home or in region.
    • Provides required documentation and reports, both clinical and administrative. Participates in case consultation.
    • Consults with parents, caregivers, referral sources, and other agencies.
    • Provides transportation for client when necessary.
    • Creates and implements psycho-education groups.
    • Promotes and interfaces with inter-agency resources as appropriate to caseload and client/family plan.
    • Maintains a flexible schedule according to client needs.
    • Completes clinical and administrative paperwork accurately and in a timely manner.
    • Maintains the client medical record and ensures contents are accurate.
    • Performs other duties in support of the Wraparound process.
    • Attends appropriate community meetings.
    • Displays regular and reliable attendance. Reports to and departs from work on time, as scheduled, and accurately reflects all time on time card.


    Education/Experience Requirements



    • Master’s degree in social work, counseling, psychology, or related behavioral science.
    • Considerable knowledge of child development.
    • Minimum two years experience working with emotionally disturbed children, youth and their families in a mental health setting.
    • Experience in documentation of records and data collection.
    • Bilingual, Fluent in English and Spanish.
    • Excellent clinical skills – engaging, strength-based approach. Ability to create and implement individual case plans
    • Experience in individual, group, and family counseling.
