  • Program/Department Description

    Our MHS Contra Costa Wraparound Services Program will provide intensive, coordinated, and community-based services to 29 unduplicated eligible referred children, youth, their families, and caregivers annually. Those referred will follow a four-stage model of Engagement, Planning, Implementation, and Transition as they work toward permanence, safety and well-being resulting in the least restrictive placements for at-risk and vulnerable children and youth. Services will be provided by a team including Facilitators, Wrap Coaches, Family Support Specialists and graduate student interns.

    Position Description

    The Facilitator develops a supportive and helping relationship with assigned high-risk youth and their families that follows the Wraparound philosophy and approach.  The Facilitator ensures that clients receive the full spectrum of services in a seamless manner and that they receive the optimal level of care needed to maximize functioning and minimize the need for higher levels of care. The Facilitator is responsible for overseeing the clinical services provided and is responsible for the client’s medical record. The Facilitator serves as the hub of the Wraparound team and completes intake, behavioral health and discharge assessments.

     

    RESPONSIBILITIES

    • In collaboration with the youth’s team members and family, serves as the main contact of the Wraparound process, providing team-based services in an in-home and community-based environment.
    • Makes initial contact with clients and provides strength-based individual and family or group mental health services, in the home or on site, including assessment, treatment planning, Wraparound meetings, case management services, and crisis intervention.
    • Conducts and completes Behavioral Health Assessments and co-creates with the team and family and completes individual Client Plans.
    • Completes Medi-Cal documentation (progress notes, assessments, client plans, and discharge summaries) and billing per County and program standards in the Electronic Health Record.
    • Designs, plans, and organizes individualized clinical services for each client and family.
    • Provides case management services for the client and family in the home or in the community.
    • Actively participates in case consultation and treatment team meetings.
    • Transports clients when necessary in own vehicle.

    Education/Experience Requirements

    • Master’s degree in Social Work, Counseling, Psychology, or related behavioral science.
    • Minimum two years’ experience working with children, youth and their families in a mental health setting.
