  • Program/Department Description

    Our MHS Contra Costa Wraparound Services Program will provide intensive, coordinated, and community-based services to 29 unduplicated eligible referred children, youth, their families, and caregivers annually. Those referred will follow a four-stage model of Engagement, Planning, Implementation, and Transition as they work toward permanence, safety and well-being resulting in the least restrictive placements for at-risk and vulnerable children and youth. Services will be provided by a team including Facilitators, Wrap Coaches, Family Support Specialists and graduate student interns.

    Position Description

    The Family Support Specialist develops a supportive and helping relationship with assigned parents and families within a Wraparound setting. The Family Support Partner helps families develop their formal and informal support network and provides rehabilitative services to parents and caregivers to improve parenting skills, self-advocacy, and self-sufficiency. The Family Support Partner actively participates in Wraparound meetings to bring forth the voice of the family members.

     

    RESPONSIBILITIES

    • Provides one on one and family rehabilitative mentorship, support services, and other interventions for direct care to parents, caregivers, and other support persons based on client and family needs identified in Wraparound meetings.
    • Develops a supportive, nurturing relationship with parents/caregivers and families.
    • Shares personal experience and expertise in navigating systems and programs to support parents, caregivers, and other support persons.
    • Interfaces with and participates in wraparound treatment teams with public systems, providers, and families.
    • Provides and models supportive parenting training to parents and/or caregivers.
    • Provides training to parents and/or caregivers on other related topics.
    • Provides advocacy for child, parents, and families.
    • Assists families in caring for the educational needs of the child.
    • Transports clients when necessary in own vehicle.

    Education/Experience Requirements

    • High School diploma or GED.
    • Minimum one-year experience working with children, youth and their families in a mental health setting.
    • Life experience as a caregiver/parent to a youth in the social services system.
