  • Program/Department Description

    MHS’ Contra Costa ACTiOn Team provides Assisted Outpatient Treatment (AOT) to individuals that qualify for AB1421/Laura’s Law. The program provides both voluntary and court-ordered services through an Assertive Community Treatment (ACT) model for up to 75 eligible adults in Contra Costa County. All services meet the guidelines described in AB 1421 (Laura’s Law) while respecting the choice, autonomy and dignity of individuals struggling with symptoms of serious mental illness and/or co-occurring substance abuse disorders.

    Position Description

    The  Registered Nurse will work with clients in Assisted Outpatient Treatment (AOT) services and subsequent Assertive Community Treatment (ACT) for Laura’s Law/AB 1421 eligible adults with serious mental illness in Contra Costa County. The Registered Nurse is responsible for monitoring the general psychiatric and physical health of all AOT ACT program clients. The Registered Nurse dispenses and accounts for all medication as ordered by the Program Psychiatrist. 



    • Provides service coordination for a shared group of clients.
    • Assumes responsibility for developing, writing, implementing and evaluating overall treatment goals in conjunction with the treatment team.
    • The majority of AOT ACT services are provided in the community and client residences, and requires client transportation in personal and/or company vehicles.
    • Provides on-call, after-hours, and weekend/holiday services as required by contract.
    • Provides individual supportive counseling and symptom management.  
    • Conducts comprehensive assessment of psychiatric history and physical health needs. 
    • In collaboration with the treatment team, participates in the organization and management of medication coaching and delivery to clients, as well as integrating medication administration into treatment plans.
    • Dispenses and accounts for all prescribed medication.  
    • Assists in administering Patient Assistance Program.
    • Completes all charting and documentation as contractually required and in a timely manner as dictated by program requirements.
    • Ensures all medical policy and procedures are followed.
    • Assists Psychiatrist in administering and evaluating medications for their effectiveness, side effects, and complications and arranging for lab work according to protocol.
    • Participates in and represents program at Medication Monitoring Committee Meetings.
    • Acts as liaison with Program Psychiatrist to obtain corrections to documentation and adjustments to procedures as needed.
    • Introduces clients to CommonGround software, assisting clients in completing the CommonGround Health Report before medical office visits.
    • Serves in a mentoring role to staff and interns regarding pharmacology and patient care.
    • Complies with all MHS and program rules, policies and procedures including HIPPA standards.
    • Demonstrates commitment to professional growth by attending in-service trainings and staff development programs.
    • Demonstrates adequate care and responsibility for equipment and ensures facilities upkeep and safety.
    • MHS requires all employees to attain a minimum degree of training and knowledge in cultural competency, co-occurring and gender responsiveness.  
    • Participates in the Injury and Illness Prevention Program.
    • Provides input towards policy and procedure design, revision, and deletion.

    Education/Experience Requirements

    • Nursing degree from an accredited college or university.
    • Minimum three years experience or equivalent treating psychiatric and dually diagnosed populations.
    • Current licensure as a Registered Nurse in the State of California.
    • Knowledge of Assisted Outpatient Treatment and Assertive Community Treatment model as it relates to client care.
    • Fluency in English required; Fluency in Spanish, Vietnamese, Tagalog, or Arabic preferred.  
    • Possesses skills necessary to provide effective counseling, therapy and clinical services, and the ability to successfully work as a member of a treatment team.
