    Strength Based Case Management North provides a wide range of services to assist individuals who suffer from a major mental illness or co-occurring disorders. We provide assistance and care coordination from evaluation to discharge planning. Our recovery centered services provide ongoing interventions, which focus on resolving problems through psychosocial rehabilitation and resource management.

    Position Description

    The Case Manager coordinates care for clients and assists them in accessing services and resources that will help maintain them in the community in the least restrictive environment. The Case Manager also provides supportive individual and group counseling. The Case Manager act as liaison between treatment providers and the client. He or she assists clients with the development of social and community support systems that promote stability and enable the client to function at the highest level possible. The Case Manager offers their extensive knowledge of community resources and support as a primary means of assisting each client to reach their rehabilitation and recovery goals. 

     

    RESPONSIBILITIES

    • Conducts comprehensive Strength Based intake assessments. 
    • Coordinates the full range of client’s needs. Provides brokerage, linkage and advocacy to ensure resources and services are accessed in a timely manner, providing community based services in client’s residence as needed.
    • Conducts individual, group counseling including crisis intervention, and oversees psychoeducational groups, family groups. 
    • Complies with all charting and documentation requirements per contract, funding agency, and MHS policy. 
    • Develops and maintains a caseload of approximately 25 clients. 
    • Collaborates with Public Conservator as needed on behalf of clients. Accompanies clients when necessary to needed services or resources. 
    • Initiates contacts with clients as often as needed to support individuals in their journey of recovery and to prevent decompensation. Provides pre-crisis and crisis planning and crisis intervention. 
    • Completes all charting and documentation as contractually required and in timely manner as dictated by program requirements. Maintains a daily log of client contacts.
    • Maintains contact with assigned treatment providers and families to continuously appraise client recovery. 
    • Prepares critical care issues for discussion during supervision, treatment team meetings and Strength Based Group Supervisions. 
    • Provides on-call services as required by contract.  Collects necessary client demographic data and enters it on the MHS Treatment Team Services Data Collection Form.  Provides information about the program to community agencies connected to the mental health services system in order to educate them about the program. 
    • Majority of work is conducted in the field.

    Education/Experience Requirements

    • Master’s degree in a behavioral science and at least two years’ experience providing case management services to individuals with mental illness.
