    Our North Star Assertive Community Treatment (ACT) program is dedicated to the treatment and recovery of adults living with a serious mental illness in San Diego’s North County. Our clients have been homeless or are at risk of homelessness and may also have a co-occurring substance abuse disorder. We offer recovery focused wraparound services, and help our clients become more productive community members, providing services that build on their strengths and further their recovery.

    Job Summary and Responsibilities

    The PT Temporary case manager is responsible for BHA's, Client Plans and overall care coordination. This position is expected to last through March 2019.

     

    RESPONSIBILITIES:

    • Provides case management for an assigned group of clients including coordinating and monitoring activities of the treatment team.
    • Develops, evaluates, and implements treatment goals and plans in conjunction with the treatment team.
    • Provides individual supportive counseling and symptom management.
    • Performs psychosocial assessments, and Alcohol and Drug screenings.
    • Coordinates the full range of client’s needs. Provides brokerage, linkage and advocacy to ensure resources and services are
      accessed in a timely manner, providing community based services in client’s residence as needed.
    • Conducts individual counseling including crisis intervention, and oversees psychoeducational and family groups.
    • Complies with all charting and documentation requirements per contract, funding agency, and MHS policy .
    • Collaborates with Public Conservator as needed on behalf of clients.
    • Completes all charting and documentation as contractually required and in timely manner as dictated by program requirements.
    • Prepares critical care issues for discussion during supervision and treatment team meetings.
    • Provides on-call and after-hour services as required by contract. Maintains a daily log of client contacts.
    • Collects necessary client demographic data and enters it on the MHS Treatment Team Services Data Collection Form.
    • Provides information about the program to community agencies connected to the mental health services system in order to educate them about the program.
    • Complies with all MHS and program rules, policies and procedures including HIPAA standards.
    • Ensures facilities upkeep and safety.
    • Familiar with and understands Mental Health Systems, Inc. Personnel Policy manual and follows all established personnel policies.
    • Participates in the Injury and Illness Prevention Program and follows guidelines that promote workplace safety.

     

    Qualifications

    • Master's degree in human services required.
    • BBS registration required.
    • Licensure preferred
    • Fluency in English required; fluency Spanish, Tagalog, Arabic, or Vietnamese preferred.
